Public Forms
Materials Request Form - AzNN Community & Supporting Partners
Community and Supporting partners may order written materials developed or obtained by the Arizona Nutrition Network for use in their nutrition education or outreach activities targeting SNAP participants and eligibles. There is no minimum amount, but very large orders may require justification from the partner (for example, 5,000 copies of "Fun Food News" to be given out in 200 classrooms in 20 low-income schools). If Community and Supporting partners wish to order incentive items, they may work with their Contributing Partners to order these items.
Materials Request Form - DES/WIC Offices
DES and WIC offices may order materials developed or obtained by the Arizona Nutrition Network for use in their office waiting areas and outreach activities targeting SNAP participants and eligibles. There is no minimum amount, but very large orders may require justification from the office (for example, 5,000 copies of "Fun Food News" to be given out at a job fair).
